Health Outreach Foundation hosts weekly clinics in conjunction with Hope Faith Ministries and Artists Helping the Homeless. We offer primary care visits for appointments as well as walk in patients. We partner with volunteer physicians and nurses to provide free health services both in person and via Telehealth.
Our goal for this program is to provide a primary care service and, where possible, offer treatment for chronic conditions such as hypertension and diabetes. In addition to these services we also host prescription service (free to our patients) and have referral relationships for our more urgent and complex patients. We also help our patients navigate referrals for any kind of labs their provider orders including blood work, MRI scan, Xrays, etc.
Our patients need medical care and assistance.
We are there for them when they need us most.
